To register with the practice, you must live within our practice area (see ‘Practice Area’ tab) and be eligible for NHS treatment.
If you are entitled to register at the practice, on arrival at the surgery you will be requested to complete a Registration Form and you must provide two forms of identification.
The first form of identification must include a Photo proof identification with date of birth and full name (e.g. passport or driving licence).
The second form of identification must include your name and current address (e.g. utility bill or other official document or a note from your employer stating permanent residence).
You will also be requested to fill in a New Patient Questionnaire.
You may wish to print off both the Registration Form and New Patient Questionnaire, fill them in and bring them with you on your first visit to the practice from the links below;

Please allow two working days for your registration to be confirmed.
We recommend you register for online access where you can book appointments, order repeat prescriptions and access your medical record.
Please print and complete the following Online Services Registration Form and bring it with you to the surgery where you will then be issued with a registration letter.

Online Access Guidelines & FAQ's
Dapdune House Surgery does not participate in the Choice of GP Practice Scheme, and therefore we are not able to register out-of-area patients, for the following reasons;
Our existing patients have made clear that their highest priority should be to improve availability of appointments. Registering patients from outside our practice area will make availability of appointments worse.
We believe that whilst this new scheme might provide benefits in terms of convenience of primary care, it risks offering poor continuity of care just at the time when the patient is most likely to need it.
A patient not registered here can, in any case, always be offered treatment in an emergency under the ‘immediately necessary’ banner where we are required to give primary medical services to any person to whom we have been requested to provide treatment owing to an accident or emergency at any place in our practice area. These appointments can only be sanctioned by a Doctor.
